What is a UAP certification?
Unlicensed assistive personnel (UAP) are paraprofessionals who assist individuals with physical disabilities, mental impairments, and other health care needs with their activities of daily living (ADLs).
What does UAP stand for in nursing?
ANA’s Principles for Delegation by Registered Nurses to Unlicensed Assistive Personnel (UAP) is designed to provide overarching principles for practice in situations across the continuum of care where registered nurses delegate tasks to unlicensed assistive personnel.
What is a UAP in healthcare?
AWHONN recognizes that unlicensed assistive personnel (UAP) also known as nursing assistive personnel (NAP) can contribute as members of the healthcare team under the direction of the professional registered nurse, who is ultimately responsible for the coordination and delivery of nursing care to women and newborns.
What are UAPS allowed to do?
(Examples include but are not limited to: bathing, feeding, ambulating, vital signs, weight, assistance with elimination, maintaining a safe environment.) Tasks which are judged by the direct care RN to not require the professional judgment of an RN may be assigned.
Can UAP take vitals?
A UAP can perform vital signs on a stable patient. However, initial vital signs should only be performed by the RN.
Can UAP measure wound drainage?
Measurement of drainage within a Hemovac drain may be safely delegated to the UAP. This is considered a routine task and is within the scope of practice of the UAP.
Can a UAP feed a client?
For example, a UAP may be allowed to feed a patient but if a high-risk stroke patient requires feeding, the nurse must first assess if the patient can safely swallow. The UAP can’t make that determination, and the nurses are expected to foresee possible harm to patients before delegating tasks.
Can a UAP monitor intake and output?
A UAP can document intake and output on a patient. If a patient is on strict orders to strain all urine then have the nurse perform the action if assessment is needed.
Can UAP empty Foley bag?
Obtaining urine from an indwelling urinary catheter tubing is not a simple specimen collection. This is a sterile procedure. The nurse must do this task. The UAP cannot irrigate catheters.
Can UAP check urine output?
Can you monitor a UAP?
Don’t delegate tasks that require specialized knowledge or complex observations, such as monitoring a patient with chest pain. UAP, licensed practical nurses (LPNs), and licensed vocational nurses (LVNs) can collect patient data, but only the registered nurse can interpret data.
Can a UAP suction a tracheostomy?
Suction the tracheostomy. Oral care (for a stable patient with a tracheostomy) can be delegated to UAP. A registered nurse would be responsible for assessments (e.g., checking the stoma for skin breakdown) and tracheostomy suctioning and care.
